NYCMG Hat/Pin Order Form

To order a FDNY hat with a NYCMG pin :

Click HERE to see / download an order form (a WORD doc) to send to Jerry Rosenberg at 12 South Oaks Blvd, Plainview, NY 11803 with $15 per hat/pin combination.

We need the forms filled out in advance  to bring the correct number of hats/pins.

All proceeds will benefit the WTC charity, the New York Police and Fire Widow's and Children's Benefit Fund.

This is a NYCMG sponsored activity and is not officially sponsored by national CMG. NYCMG is responsible for the collection of all monies for this benefit.
